Start each personnel file with an index. It should tell you which records are present, which need attention, and who owns the next step. This guide uses Pennsylvania’s center rules as its reference; group and family homes should use the corresponding rules for their program.

What belongs in a center personnel file?

55 Pa. Code § 3270.192 describes personnel-record contents. Its categories include contact information; age, experience, education, and training verification; health-assessment documentation; clearance-related records; and two written nonfamily references. Read the provision and applicable related rules to determine what evidence is required for each employee.

Do staff health assessments expire every year?

For people covered by the center requirement, § 3270.151 specifies an assessment within 12 months before initial service and every 24 months afterward. It also describes validity conditions and exceptions. Do not apply a blanket annual deadline to everyone. Read the full rule and confirm how it applies to the person’s role and circumstances.

Track courses alongside the evidence

Make a file review repeatable

  1. Confirm the roster. Start with the people currently working in your program and identify who is responsible for each file.
  2. Check the index against the records. Open the actual document rather than relying on a checked box from an earlier review.
  3. Write a specific request. “Need updated document” is less useful than naming the exact item and the next action.
  4. Track the follow-up. Record when you asked, who is responding, and when you will check again.
  5. Close the loop. When an item arrives, review it, update its status, and record the applicable next date.

Use role-appropriate access. A shared team task list can say that a record needs review without exposing the underlying medical or clearance document.

Use official forms

A spreadsheet reminder is an organizational choice, not proof that a requirement has been met. Store the source supporting each deadline with your checklist, and revisit it when guidance changes.
